Registration Process
Here is what you Need to do to Register with our Office:
The first step is to complete the Registration Form. You may also complete the . Our registration form covers a broad range of information. Some things may apply to you, and some items may not. You may skip any questions you feel do not apply to you. There are no right or wrong answers (This form helps us understand your experience better, and which accommodations may be a good fit).
The next step is for you to provide Disability Services (DS) with verification documentation. There are three different ways for you to do this.
- Send us any previous Documentation from a licensed provider stating your disability, diagnosis, and/or symptoms you are experiencing.
- Have your provider complete our Verification Form (stating your disability).
- Alternatively, you can complete our Release of Information Form. This form will authorize DS to request documentation from whoever you designate on your behalf. This form should only be filled out if you wish to release confidential information. Please note: DS cannot guarantee receipt of verification from providers.
Once DS receives both the registration form and documentation of your disability, and you are registered for classes, DS staff will schedule an accommodation meeting with you and one of our accommodation managers. We will get to know one another and discuss what accommodations may address barriers to your academic career. Accommodation meetings usually take about an hour and can be conducted in person, over the phone, or via Google Meet at your request.
After the meeting, DS will enter your accommodation eligibility into AIM. You can log in to AIM at the start of the semester to send your accommodations to your professors. You must to renew your accommodations every semester. Here are the instructions for requesting accommodations in AIM:
- Go to the DS Website.
- Click the blue button that says ""
- Sign in using your UA single sign on (Same as Canvas and UA Online)
- Near the center of the page, underneath "Accommodation Requests Fall 2025", click "Add requests for 2 classes"
- Select the classes for which you would like your accommodation, and click "continue to next step."
- Select your accommodations, and click "Submit Request."
After you do this, the next page should list the classes, the accommodation requested, along with "processing”. When you see this, your end of the process is complete, and an email will be sent to your instructors shortly thereafter.
